Professional Intervention: Multifaceted

Definition

Any combination of two or more professional, organisational, financial, structural or regulatory interventions designed to improve prescribing practices or other health provider behaviour.

Effectiveness

Twenty-nine high quality reviews (Alldred, Archer, Baskerville, Buckley, Faulkner, Flodgren, Forsetlund, Free, French, Gould, Gruen, Harvey, Jamtvedt, Laliberte, Lemmens, Lewin, Little, Ma, Moore, Morgado, Newton, O'Brien, Opiyo, Patterson, Pirkis, Reeves, Smith, Sturm, Thomas) and two key reviews (Arnold, Davey) were identified. Results from six high quality reviews (Baskerville, French, Jamtvedt, O'Brien, Smith, Pirkis) and one key review (Davey) show that multifaceted interventions are generally effective for improving appropriate care behaviours. There were mixed results from two high quality reviews that evaluated multifaceted interventions with an educational meeting component; in the Forsetlund review, this intervention was generally effective but in the Newton review it had mixed effects on improving appropriate care outcomes. The results in the Moore review were unclear to us in terms of effectivness and readers should consult the review to draw their own conclusions on the effectiveness of the intervention on appropriate care behaviours. Findings from three high quality reviews (Jamtvedt, Morgado, O'Brien) found multifaceted interventions to be generally effective for improving prescribing-related outcomes. However, mixed results on prescribing outcomes were found in two key reviews (Arnold, Davey) and one high quality review (Lemmens). One high quality review (Lewin) found multifaceted interventions to be generally ineffective for improving appropriate care behaviour and there was insufficient evidence in nine high quality reviews (Archer, Faulkner, Free, Harvey, Little, Ma, Opiyo, Reeves, Thomas) to draw any conclusions about the effects of multifaceted interventions on appropriate care outcomes. There was also insufficient evidence in twelve high quality reviews (Alldred, Buckley, Forsetlund, Free, Gould, Gruen, Laliberte, Lewin, Little, Patterson, Smith, Sturm) to draw any conclusions about the effects of multifaceted interventions on prescribing outcomes.

Summary of Overall Findings from Reviews

134 reviews that evaluated the effectiveness of multiple interventions were identified. Of these, 9/16 high quality/key reviews with a sufficient number of studies to draw conclusions found this intervention to be generally effective. One high quality review (Flodgren) found this intervention to have mixed results and one review (Moore) had unclear results.
