Reminder - CADTH Health Technology Expert Review Panel - Call for Nominations

CADTH is seeking nominations for one health care practitioner, for its Health Technology Expert Review Panel (the Panel). The Panel is an advisory body convened to develop recommendations for CADTH projects related to medical devices (includes devices, diagnostic tests, and medical, surgical, or dental procedures). Recommendations provided by the Panel will inform a range of stakeholders within the Canadian health care system.

Qualifications & Eligibility

Nominees must hold a professional degree from a recognized institution in medicine, nursing, and have experience in health services research, epidemiology, critical appraisal, or evidence-based medicine .  For more information on qualifications, please review the role descriptions for Core Members.

Given that the Panel will meet frequently throughout the year and that preparation is required prior to each meeting, nominees should be advised that a time commitment is involved.  An honorarium is paid to Panel Members in recognition of the time required for preparation and meetings.  More detail is available in the role description for the Core Members.

Who Can Submit a Nomination?

Federal, provincial and territorial jurisdictions, relevant health professional bodies, individual health and other professionals, the general public and the Panel Nominating Committee may nominate Members. Nominations may be submitted on the candidate’s behalf or interested candidates may nominate themselves.

Nomination Materials and Instructions

The following materials and instructions should be reviewed when considering or preparing a nomination:

1. Terms of Reference
2. Member role description
3. Conflict of Interest Guidelines (The Conflict of Interest Guidelines contain disclosure forms that nominees are required to complete and return as part of the application. Full disclosure is required from all nominees.)
4. Code of Conduct (Each nominee must sign and return this form.)
5. Nomination Form (Each nominee must complete, sign and return this form) 

Nomination Package

The following materials must be submitted in support of Panel nominations:

• completed nomination form, signed and dated
• nominee’s curriculum vitae
• completed and signed Conflict of Interest forms
• signed Code of Conduct form

Completed nomination packages must be submitted to CADTH, either directly by the nominator or by the nominee, no later than 4:00 PM EDT on Tuesday April 12, 2016 to the attention of Cheryl Holmes via:
